School, Sports & Camp Physicals

At PhysicianOne Urgent Care, we offer school, sports, and camp physicals. Please be sure to bring the following:

  • Any forms provided by your child’s school, sports team, or camp
  • A copy of your child’s most recent immunization record
  • Your driver’s license
  • Your health insurance card (for easy reference in case of future visits)
  • A credit card for payment

Convenient Location With Extended Hours

If you’re in need of a physical, visit PhysicianOne Urgent Care’s Ridgefield office, located at 10 South Street, just off Danbury Road. We understand that many people can’t get out of work during the normal business day, which is why we offer extended hours — we’re open from 8 a.m. to 8 p.m. on weekdays, and from 9 a.m. to 5 p.m. on weekends and holidays.
