About Camp Physicals

A camp physical evaluates a child’s overall wellbeing and underlying medical conditions to ensure he or she is healthy enough to participate in common camp activities, like canoeing, hiking, and swimming. If accommodations need to be made (for example, a child with asthma may need to have an inhaler with them at camp), they will be noted on the physical examination form.

During the exam, the practitioner will review the child’s family and medical history as well as his or her immunization record. If any vaccines are needed, we can likely administer them during the physical. The practitioner will also record height and weight, take vitals, and evaluate his or her ears, nose, throat, abdomen, lungs, heart, and major joints.

How to Prepare

It’s important to bring all important documents when visiting our urgent care center for your child’s camp physical. These include:

  • The camp physical authorization form
  • A copy of your child’s most recent immunization record
  • Your insurance card (if using health insurance)
  • Your driver’s license
