Since we’re open 365 days per year and have extended hours (we’re open until 8 p.m. Monday-Friday and until 5 p.m. on weekends and holidays), you’ll be able to visit us at a time that easily fits your schedule.

What Do I Need to Bring to a Physical Exam?

Whether you need a simple sports physical so your young child can play soccer or a more involved CDL physical to be licensed as a commercial driver, there is specific paperwork required at each type of exam. Failure to bring these items may result in our medical professionals not being able to complete your physical.

For school, sports, and camp physicals, please bring:

  • A recent immunization record for your child
  • The physical exam request form
  • Payment (the fee for these physicals is $80)

For CDL/DOT physicals, please bring:

  • A copy of the physical exam form with sections 1 and 2 filled out
  • A copy of the medical examiner’s certificate
  • Payment (the fee for this physical exam is $150)
