While not a replacement for an annual physical—which is a much more involved examination—it can give parents a good snapshot of their child’s overall well-being and development.

When you visit our urgent care center for a school, camp, or sports physical, be sure to bring the following items:

  • The physical exam form that must be filled out by a medical practitioner
  • Your child’s immunization records
  • The driver’s license and insurance card of the accompanying parent
  • A form of payment
