Patients seen at PhysicianOne Urgent Care have the ability to request medical records from their visit.
Beginning August 13, 2024, the Health Information Management (HIM) team at Yale New Haven Health will handle these requests for all PhysicianOne Urgent Care visits before and after August 13, 2024. Please visit the link below to request medical records.

Please note: As of August 13, 2024, our Connecticut and New York locations have upgraded to Yale New Haven Health's Epic medical records system. With this update, patients seen at our Connecticut and New York locations will be able to access their medical records, including test results, through their Epic MyChart account.
