At PhysicianOne Urgent Care, we offer school, sports, and camp physicals. Please be sure to bring the following:
- Any forms provided by your child’s school, sports team, or camp
- A copy of your child’s most recent immunization record
- Your driver’s license
- Your health insurance card (for easy reference in case of future visits)
- A credit card for payment
